Organizzare le Case della Comunità, tra struttura e relazioni. Una prospettiva sociologica sull’integrazione multiprofessionale come processo di cambiamento

This thesis explores the theme of multiprofessional and social-healthcare integration within the Case della Comunità in Lombardy, analyzing how this process is taking shape within the recent transformations of the public healthcare system. The aim is to understand whether and how the organizational configuration of the Casa della Comunità enables or constrains integration, by relating structural and social dimensions within a change management perspective. The research adopts a qualitative approach inspired by action research and grounded theory, and is based on multiple sources: an exploratory study with a case study, 26 semi-structured interviews with district directors, and 4 focus groups with multiprofessional teams. Thematic analysis, supported by MAXQDA software, made it possible to identify integrative relational practices enabled by the organizational structure, as well as the factors that hinder them. The sociological interpretation of the results contributes to a deeper understanding of the relational mechanisms of integration within the Case della Comunità, while also offering useful insights to guide support actions for professionals engaged in the ongoing processes of change within the Regional Health Service.

La tesi indaga il tema dell’integrazione multiprofessionale e sociosanitaria nelle Case della Comunità in Lombardia, analizzando come tale processo si stia delineando nel quadro delle recenti trasformazioni del sistema sanitario pubblico. L’obiettivo è comprendere se e in che modo la configurazione organizzativa della Casa della Comunità abiliti o limiti l’integrazione, mettendo in relazione dimensioni strutturali e sociali in un’ottica di change management. La ricerca adotta un approccio qualitativo ispirato all’action research e alla grounded theory, e si fonda su un’articolazione multipla di fonti: un’indagine ricognitiva con uno studio di caso, 26 interviste semi strutturate a direttori di distretto e 4 focus group con équipe multiprofessionali. L’analisi tematica, supportata dal software MAXQDA, ha permesso di individuare le pratiche relazionali orientate all’integrazione abilitate dalla struttura organizzativa e i fattori che la ostacolano. L’interpretazione sociologica dei risultati contribuisce ad una comprensione più profonda dei meccanismi relazionali dell’integrazione nel contesto delle Case della Comunità, offrendo al contempo indicazioni utili per orientare le azioni di supporto ai professionisti protagonisti dei processi di cambiamento in atto nel Servizio Sanitario Regionale.
